Kaplica - mauzoleum w stylu neogotyckim, zaprojektowana przez warszawskiego architekta Hugo Kudera. Zbudowana w 1904 roku na zlecenie Adeli Buchholtz, której mąż Adolf zginął w tragicznym wypadku podczas przejażdżki konnej. Obiekt położony jest na cmentarzu ewangelickim przy drodze 676 w Supraślu.
